What Is TEETH WHITENING?
Teeth whitening is a process that lightens teeth shade and thereby helps in removing stains and discoloration from tooth surface. Whitening is considered among the most sought cosmetic dental procedures because of the drastic results it brings.

When it is used ?
The outermost layer of our tooth is called ‘Enamel’ and the inner layer is called ‘Dentin’. Your genes affect the thickness and smoothness of the layer of enamel. Thinner enamel layers allow more of the colour of the dentin to be reflected. Also the smooth or rough enamel surface affects the reflection of light and thereby the colour.

Every day, a thin coating (aka pellicle) deposits on the enamel and picks up various shades of stains. The enamel also contains varying size of pores that can hold stains of different magnitude. The following are the most common reasons for teeth to get stained:
Using tobacco and its products Chronic drinking of various coloured liquids such as coffee, cola, tea and red wine Improper oral hygiene.
Aging causes consistent wear off of tooth structure thereby making teeth less bright as the enamel gets thinner and the dentin becomes darker.

It is also possible to have stains within the tooth. These are known as ‘Intrinsic stains’. For example, intrinsic stains can be seen due to excess exposure to too much fluoride during childhood while teeth are developing. Other causes include consumption of drugs such as tetracycline antibiotics. They can cause staining of a child’s teeth if taken by the mother mainly during the second half of pregnancy or by a child himself below 8 years of age when teeth are still under developmental process during these years. Trauma can also darken a tooth due to internal
injuries. Tooth whitening is found to be most effective on surface (extrinsic) stains.

How It is Done?
There are 2 main types of whitening techniques. Vital whitening is performed on teeth that are vital and has live nerves. Non­vital whitening is done on a tooth that has had a history of root­canal treatment and no longer contains a live nerve.

Vital Whitening Technique:
The most common type of vital tooth whitening makes use of a commercially available gel that is applied directly onto the tooth surface. This product has some form of hydrogen peroxide as a de­staining agent.

Tooth whitening can be done in the dentist’s office or at home based on the case selection. In­office whitening permits your dentist to use a stronger and concentrated whitening gel. A specialized light on a specific frequency activates the gel and allows the process of bleaching to happen faster.

In­office whitening usually takes about 30­90 minutes requiring 1­3 dental sittings. The number of applications will depend upon the method used, severity of your stains and the intensity of whiteness expected by the patient. Different types of stains respond in a different manner to the treatment.

For in­house whitening, your dentist will require impressions of your upper and lower teeth for fabrication of custom made mouthpieces to exclusively fit you. An ideal fit of the mouthpiece allows the whitening agent remain in contact with your teeth in a uniform manner.

At home, you are expected to fill each mouthpiece with a whitening gel provided to you by the dentist. You are required to wear the mouthpiece for prescribed amount of time for several days. Many people attain the amount of whitening they desire within a span of a week or two.
However, in some cases you may be required to pursue the treatment for four weeks or longer.

Non­Vital Whitening technique:
Vital whitening may not fetch the same results to a tooth that has had root­canal treatment as the source of stain in insidious in nature. In such a case, your dentist will adopt a different procedure that can whiten the tooth from the inside. This technique requires placing a whitening agent inside the tooth and further adding a temporary filling over it. The tooth will be left to be bleached for several days. Sometimes one application can be sufficient, or the application can be repeated until the tooth reaches the desired shade.

Associated Risks:
Whitening is unlikely to cause prominent side effects, although sensitivity can be present for a short while. Patients commonly experience mild gum irritation as well. It is highly advised for women to not undergo teeth whitening during pregnancy. The effect and influence of the whitening agents on the fetus during development is not known.
